By agreeing to attend The Dizziness Physio consultation, you agree to the below terms and conditions.
I consent to the clinical assessment and treatment modalities provided by The Dizziness Physio
I understand that clinical assessment and treatment may require an appropriate level of clothing removal in order to carry out the appropriate treatment
I am aware that management of my complaint may cause unpleasant sensations which include, but are not limited to: temporary soreness, bruising, headache, nausea, dizziness and possible exacerbations and/or aggravation of underlying conditions
I understand that every effort will be made to make my experience as comfortable as possible and I will communicate any concerns or discomfort that I experience
I acknowledge that I have the right to decline treatment at any time
I do not expect the practitioner to be able to anticipate all potential risks and complications associated with the proposed care
I accept that for effective treatment and management of my complaint I may be required to participate in self-education, rehabilitative and therapeutic exercise
I have informed and will continue to inform the clinic of any illnesses, injuries, medical conditions or procedures and any other information regarding my health
I authorize the release of all necessary information to my primary care provider and/or referring physician
I acknowledge that I am required to give 24 hours notice of cancellation or rescheduling of an existing appointment or I will be required to pay the consult fee
Declaration: I hereby acknowledge that I am aware of and understand the terms and conditions of The Dizziness Physio’s treatment and I agree and understand that the information I have provided is accurate, current and will be confidential in accordance with the National Privacy Principles and the Privacy Act (2001)

The Dizziness Physio is committed to implementing and promoting a Privacy Policy that will ensure the privacy and security of your personal information and abides by the National Privacy Principles.
We will collect, maintain and use your personal information only for the primary purpose of providing you with advice and services.
We may also use your personal information for other purposes related to the primary purpose such as administration, planning, product development and research purposes.
